FSHES Technology Department 2020-2021 Parent/Student iPad Agreement Form
Important note: Due to the COVID-19 pandemic and restrictions placed on having students and parents on campus, we are moving our hard-copy form to this online Google Form. Please read this in its entirety before electronically signing.

FSHISD PreK and Kindergarten students may be issued an iPad for their educational use. With the acceptance of the iPads, FSH Elementary School students and their parents take on certain responsibilities. Acceptance of and adherence to these responsibilities will result in a rich technologically-enhanced learning environment for all involved.

It is our belief that if reasonable precautions and care are taken in the use of the iPad, it should not experience physical damage. Each student and parent is asked to read & complete this form carefully. The parent will select "I agree" next to each statement that follows, and sign your name at the bottom of the form.  We are asking for your email and phone number in order to contact to you about your child's iPad if needed. Please know that this phone number and email will not be shared.

Please read the following statements carefully and check the box that says "I agree".
1. We understand that the iPad and its accessory equipment are the property of the Fort Sam Houston Independent School District. *

2. We understand that the student, with the support of the parent, is responsible for the daily care and maintenance of the iPad. *

3. We understand the student must abide by the rules and regulations of the Student Responsible Use Agreement for Technology Resources. Failure to abide by this policy may result in disciplinary action and/or confiscation of iPad. *

4. I understand that when my child returns for in-person instruction or at the end of the school year, this device will be returned to the campus on days designated and communicated to you by the tech department. *

5. We understand that we must immediately report all iPad damages, or the theft/loss of the iPad as quickly as possible. *

6. We understand that loss of the device is not covered and, if lost, we must pay the $500 replacement fee. *

7. We understand that a cracked screen is not covered and, if cracked, we must pay the $100 repair fee. *

8. We understand that the student's name MUST be on the iPad and charger at all times using the labels provided at distribution. *

9. We understand that the iPad case MUST remain on the iPad at all times.   *

10. We understand that the iPad is managed by the district through a district iTunes account.  We will not be able to sign into a personal iTunes account or add apps that have not been specifically purchased and pushed out by the district. *
