FSHES Technology Department 2020-2021 Parent/Student Chromebook Agreement Form
Important note: Due to the COVID-19 pandemic and restrictions placed on having students and parents on campus, we are moving our hard-copy form to this online Google Form. Please read this in its entirety before electronically signing.

FSHISD students may be issued a Google Chromebook for their educational use. With the acceptance of the Chromebooks, FSH Elementary School students and their parents take on certain responsibilities. Acceptance of and adherence to these responsibilities will result in a rich technologically-enhanced learning environment for all involved.

It is our belief that if reasonable precautions and care are taken in the use of the Chromebook, it should not experience physical damage. Each student and parent is asked to read & complete this form carefully. The parent will select "I agree" next to each statement that follows, and sign your name at the bottom of the form.  We are asking for your email and phone number in order to contact to you about your child's Chromebook if needed. Please know that this phone number and email will not be shared.

Use, Proper Care, Precautions, & Personal Responsibilities of Chromebook
Please read the following statements carefully and check the box that says "I agree".
1. We understand that the Chromebook and its accessory equipment are the property of the Fort Sam Houston Independent School District. *

2. We understand that the student, with the support of the parent, is responsible for the daily care and maintenance of the Chromebook. *

3. We understand the student must abide by the rules and regulations of the Student Responsible Use Agreement for Technology Resources. Failure to abide by this policy may result in disciplinary action and/or confiscation of Chromebook. *

4. I understand that when my child returns for in-person instruction or at the end of the school year, this device will be returned to the campus on days designated and communicated to you by the tech department. *

5. We understand that we must immediately report all Chromebook damages, or the theft/loss of the Chromebook as quickly as possible. *

6.  We understand that accidental damage is covered by the school, but we must pay a $30 handling fee for each incident/repair. *

7. We understand that loss of the device is not covered and, if lost, we must pay the $350 replacement fee. *

8. We understand that theft of the device is not covered and, if stolen, we must pay the $350 replacement fee. We further understand that the school offers theft insurance through a third party vendor and that enrollment information can be found on the campus websites under the parent tab and 1-1 Chromebook Program. *
